抄録

A 51-year-old man noticed subcutaneous tumors in his right axilla one and half months ago. The tumors were removed and examined histopathologically in the department of surgery. Initially, he was diagnosed as sarcoidosis and consulted to our department. On our first examination, two fistules were found in the operative wound, and mycobacterium tuberculosis was cultured from the exudate of the fistula. Additionally, several reddish papules were distributed on his upper arms. A histological section of the papule showed inflammatory cell infiltration around the dermal arterioles and the degeneration of their walls. After one month of antituberculosis therapy, the axillar fistules and the papules on the upper arms healed. We diagnosed the axillar lesions as scrofuloderma from the clinical and histopathological findings, the rapid response to the antituberculosis therapy and the culture of mycobacterium tuberculosis, and considered that the papules were papulonecrotic tuberculid in the early stage which followed the scrofuloderma.
