We investigated whether the therapeutic effect using a dextran-sulfate cellulose (DSC) column might be influenced by variations in plasma lipid composition. LDL-apheresis using the DSC column was performed in patients with type IIa homozygous familial hypercholesterolemia (FH), type IIa heterozygous FH, and type V hyperlipoproteinemia. In all patients, the removal rate of lipids was the highest for LDL. Among apolipoproteins, apolipoproteins B and E were removed selectively by DSC column in all patients. For plasma separation, hollow fibers were less effective in type V hyperlipoproteinemia than in FH.
After LDL-apheresis using the DSC column, the attacks of abdominal pain noted before LDL-apheresis in a patient with type V hyperlipoproteinemia were improved, and regression of tuberous and tendinous xanthoma was induced in patients with FH.
The data suggest that, by the use of the DSC column, a therapeutic effect can be induced independent of variations in lipid composition. Therefore, LDL-apheresis is equafy effective in patients with type IIa and type V hyperlipoproteinemia.
