Two cases in which amyloid casts of β2-microglobulin origin were found in the renal tubuli are presented. The first case was a 15-year-old female with chronic renal failure due to polycystic kidney who had been on hemodialysis for a period of 4 months. Her left kidney had been removed for renal transplantation. Amyloid casts were found in her renal cortical tubuli and identified as of β2-MG origin by immunohistochemical techniques. The second case was a 71-year-old female with chronic renal failure due to chronic glomerulonephritis who had been on hemodialysis for a period of 8 years. Her left kidney had been removed because of a Grawitz's tumor. Amyloid casts were also found in her renal cortical tubuli.<br>While amyloid casts are very rarely found in renal tubuli, dialysis-related amyloid deposition is frequently found in bone and synovial tissue, in vascular walls in the heart, lung and kidney, and in the myocardium. With regard to the location of amyloid, casts in luminal space are clearly different from those deposited in tissue. Through consideration of the mechanism of amyloid fibril formation from β2-MG, these findings may play a contributory role in resolving the problems of dialysis related to amyloidosis.
