The patient, a 55-year-old male, was diagnosed as having PSS in 1995, visited our department on 14 May 1996, and was hospitalized in June 1996 because of leukopenia. Interstitial pulmonary fibrosis was detected by examination at hospitalization, and treated with steroid. During the treatment, pericardiocentesis was performed because of stagnation in the pericardial cavity. Pericarditis associated with PSS was suspected, but adenocarcinoma was detected by cytodiagnosis. Its primary lesion was not found by general examination, but tumor masses were detected at autopsy in the epicardium, upper mediastinal space and aortic arch, which were considered to be infiltration from the lung. There have recently been a number of reports on the relationship between PSS and internal malignancy, lung cancer in particular, and this complication should be clinically noted.
