抄録

The omental flap is useful for reconstructive surgery. It is used extensively to cover difficult, infected wounds or complex cavities. We used a composite gastric seromuscular patch and an omental flap for reconstruction of the middle skull base.<br>A 58-year-old woman underwent a right frontotemporal craniotomy, an orbitozygomatic osteotomy, and removal of a skull base meningioma, by an infratemporal approach. The dural defect was closed using free femoral fascia. The mucosal defect of the posterior nasal cavity was closed using the composite gastric seromuscular patch and an omental flap, and the infratemporal dead space was filled with the omental flap. The right gastroepiploic artery was anastomosed to the superior thyroid artery, and the right gastroepiploic vein was anastomosed to the external carotid vein. The postoperative course was uneventful.<br>The composite gastric seromuscular patch and the omental flap were useful for reconstruction of the middle skull base, not only because it has a long pedicle of vessels but also because it can provide an immediate watertight seal.
