- Case Report of 21-year Postgastrectomy Retrograde Intussusception of the Jejunum Occuring Distal to a Roux en-Y Anastomosis

A 74-year-old woman, who had undergone a total gastrectomy 21 years previously presented with nausea, vomiting and pain in the umbilical area. Following a diagnosis of intestinal obstruction, she was referred to the emergency outpatient department of our hospital. On admission, a large, fist-sized tumor extending from the left umbilical side to the left lower part of the abdomen was identified; tenderness and muscular defense in this area were also noted. An abdominal CT scan revealed a “target like sign” on an image of the above area. Intestinal obstruction caused by intussusception was diagnosed, and a laparotomy was performed. The intraoperative findings revealed a retrograde intussusception. Invaginated bowel progressing in an antiperistaltic direction was present on the anal side of the postgastrectomy Y-anastomosis. After a manual reduction, the absence of organic disorders in the invaginated jejunum was confirmed, and the surgery was completed without performing intestinal ablation. No postoperative complications occurred, and the patient was discharged from hospital 10 days after admission. No subsequent relapses have been detected, and the patient remains in good condition.
