Causes of dysphagia can be divided into two major groups: static disorders and dynamic disorders. The former implies disorders of the food way itself, such as, mass, stenosis, fistula etc., whereas the latter implies disturbances in the action of transporting bolus, for example, paralysis, spasm, or incoordination of one or more muscles. The present paper describes surgical treatments for dysphagia due to dynamic disorders in order to call attention of head and neck surgeons to the fact that such a dysphagia can be successfully treated.<BR>Several surgical techniques are effective if they are indicated adequately. They are classified into five groups according to the purpose of operation:<BR>(1) Surgeries to correct velopharyngeal incompetence, such as, pharyngeal flap operation,<BR>(2) Surgeries to correct glottic incompetence, such as, mediofixation of the vocal cord and intracordal injection,<BR>(3) Surgeries to strengthen the paretic pharyngeal wall including partial removal of the paretic wall,<BR>(4) Cricopharyngeal myotomy whose purpose is to eliminate resitance at the entrance of the esophagus, and<BR>(5) Surgeries to help elevation of the larynx, such as, infrahyoid myotomy and upward fixation of the hyoid bone and/or the thyroid cartilage.<BR>We have treated seven cases: one case of cricopharyngeal spasm, three of cerebrovascular accident, and three of amyotrophic lateral sclerosis. In all the cases, the surgical operation(s) was more or less effective.
