Mandibular reconstructive surgery is an indispensable operative procedure for a patient who undergo a radical surgery for oropharyngeal cancer to maintain their speech, swallowing, respiratory and masticating function. A serious cosmetic problem of appearance would also arise after the craniofacial operation associated with major mandibular defects. Primary mandibular reconstruction frequently require provision for interoral lining, tongue and as well as lateral wall of mesopharynx. Scapular osteocutaneus free flap is preferred for reconstruction of mandibular, because scapular osteocutaneus free flap has great flexibility in three-dimensional reconstruction and can be combined with other flaps based on the circumflex scapular artery.<br>In this study, the masticating function among 26 cases of mandibular reconstruction by scapular osteocutaneus flap were quantitated using dental prescale occluder system. Postoperative mandibular mastication depended on the number of remaining natural teeth, the presence of adhesion between denture and the mobility of remaining tongue. Good mastication, expected in the case of good adherence of denture and remaining natural teeth, was obtained, since resection of swallowing muscles, for example, root of tongue and lateral wall of mesopharynx, resulted in a lowering of swallowing function, mastication on the reconstructed mandible was possible, but deglution was difficult in those cases. Therefore, osteointegrated implants were adequate for total mandibular reconstruction, but inadequate for anterior or anterolateral mandibular reconstruction.
