Electron Microscopic and Immunohistochemical Observation of the Mesangial Deposition in Smoldering Glomerulonephitis

Abstract

Renal biopsy specimens obtained from thirty patients who had chance proteinuria and hematuria in spite of normal renal function who observed by electron microscope. Their morphological changes were so-called smoldering glomerulonephritis classified by Kinoshita. In 23 patients the electron high dense deposits were present in the mesangial matrix. In several cases the histological changes were hardly defined as abnormel. IgA deposition in the mesangum was observed in 14 Patients among 15 patients with positive immunof luorescent findings. We tried to mesure the maximum diameter of the largest mesangial deposit and compered it to the width of the basament membrane. These was no particular relationship between the size of the maximum mesangial deposit and the severity of the glomerular lesions. The electron microscopical immunoperoxidase method was done in one patient, ant it was clarified the IgG was localized within the electron high dense deposits in the mesangium. The most striking feature that IgG was observed in the intercellular space between the mesamgial cells. This fact displayed the specific Junctional pathway from the glomerular capillary luman to the paramesangial area and suggested that immune complex was accumulated into the paramesangial area through its way.
