  • Five-year follow-up of ocular deviation and stereopsis in bilaterally pseudophakic patients

Purpose: To investigate long-term changes in ocular deviation and stereopsis in patients implanted with a monofocal intraocular lens (IOL) in both eyes.<BR>Subjects and Methods: Subjects were 88 exophoria patients over 50 years of age (19 males, 69 females; average age, 68±8 years; average axial length, 23.9±0.9 mm) who were scheduled for bilateral phacoemulsification and monofocal IOL implantation. Inclusion criteria were: no vertical deviation, phoria maintenance with an exophoria angle of 2 PD to 16 PD at near and an exophoria angle of 0 PD to 14 PD at distance, postoperative visual acuity of 1.0 or better. Before surgery, at 1, 3 and 6 months and 1, 2, 3, 4 and 5 years after surgery, with corrected refractions ocular deviation was measured by alternate prism cover test and near stereopsis was measured by the Titmus stereo test. <BR>Results: Marked changes in ocular deviation at near were observed at 5 years after surgery. Of 88, 10 (11.4%) patients shifted from exophoria to intermittent exotropia or exotropia and experienced diplopia. Most of these patients had a preoperative near exophoria angle greater than 12 prism diopters (PD). No changes in ocular deviation at far were observed. At 5 years after surgery, the median near stereopsis was 50 seconds of arc (arc-sec) with 75 (85%) patients showing normal stereopsis under 100 arc-sec. <BR>Conclusion: In bilaterally pseudophakic patients with a near exophoria angle greater than 12 PD before cataract surgery, the possibility of postoperative diplopia should be considered and a long-term follow-up is important.
