Purpose: Highly myopic strabismus is considered acquired progressive strabismus that usually occurs after one's middle age. We encountered a patient who developed the condition at the age of ten and evaluated the pathogenic mechanism in this case.<BR>Case: At the age of 10, the patient suddenly developed esotropia and diplopia in the left eye and consulted Shiga University Hospital, where he was diagnosed with esotropia of 20 prism diopters (⊿) and myopia of -4.50 diopters (D). Before entering high school, he had consulted Ozaki Eye Clinic due to severe headache and diplopia and had since been observed. The symptoms were once alleviated but recurred before the subject took the college entrance examinations. Due to exacerbation of the symptoms, he consulted Shiga University Hospital again and was recommended to undergo surgery. Because of a change of residence, he was referred to our hospital. On his first visit to our hospital in May 2011, the patient exhibited esotropia of 35⊿ in both far and near visions and refractions of S-9.00 D in the right eye and S-9.25 D Cyl-0.50 D 65° in the left eye. The axial length was 27.61 mm in the right eye and 27.79 mm in the left eye. The Hess red-green test showed bilateral abduction deficits. By MRI, the angles of the globe dislocation of 135° in the right eye and 121° in the left, nasal displacement of the bilateral superior rectus muscles, inferior displacement of the bilateral lateral rectus muscles, and thinning, deviation and rupture of the lateral rectus-superior rectus (LR-SR) band were noted. <BR>Conclusion: From the progression course of myopia and esotropia, elongation of the axial length, subluxation of the eye globe, and MRI findings, a diagnosis of highly myopic strabismus with an extremely early onset was made. Thinning of the LR-SR band was considered to be its primary cause.
