  Although strabismus surgery is often performed on patients with monocular anopsia, to our knowledge, strabismus surgery for patients with binocular blindness has not been reported. In this study, we reported a patient with binocular blindness whose mental quality of life (QOL) had been improved after exodeviation surgery. <BR>   The patient was a 61-year-old man who had lost his vision because of secondary glaucoma due to Behcet disease. He had been using an ocular prosthesis since the age of 36 after the left eye was enucleated because of ocular phthisis. Around the age of 59, his right eye started to show exodeviation and the patient suffered mentally for the cosmetic reason. The situation was resolved and his mental QOL was improved after he received a surgery for the exodeviating right eye.<BR>   In conclusion, if the patient has a strong wish, strabismus surgery performed on the blind eye could be useful to improve the patient's mental QOL.
