抄録

A 43-year-old male was admitted to our hospital with a pancreatic stone and chronic pancreatitis. Extracorporeal shock wave lithotripsy (ESWL) and endoscopic lithotomy were attempted to remove the stone but proved ineffective because the stone was impacted. Electrohydraulic lithotripsy (EHL) was then performed using the SpyGlass system, but failed to break the stone. Consequently, we recommended a surgical procedure to the patient, but he declined. Therefore, ESWL therapy was continued, eventually disintegrating the pancreatic stone, and endoscopic lithotomy was performed to remove the stone fragments. We consider that EHL cracked the stone and increased the effect of ESWL. After removal of the stone, an endoscopic pancreatic stent was inserted into the main pancreatic duct. This report describes a difficult case of pancreatic stone removal requiring long-term repeated ESWL and endoscopic lithotomy.
