Since the founding of the Japan Diabetic Society in 1958, many diabetes clinics, including the Diabetes Center at Tokyo Women's Medical College, have been established in Japan. During the 18 years between its establishment and 1976, our center has given regular treatment to both new and long-term diabetic patients once or twice a month.<BR>The purpose of this report is to describe the clinical findings of 1, 629 diabetic patients seen at our center during a 12 month period from January through December 1976. These patients underwent physical and laboratory examinations and were questioned about a family history of diabetes, smoking and drinking habits, etc.<BR>Clinical findings are as follows:<BR>1. The man-woman ratio was 1. 23.<BR>2. The peak age at examination was 70 or more, and the peak age at the onset (of diabetes) was 60 or more.<BR>3.38.4% of the patients had had diabetes for more than ten years.<BR>4. A family history of diabetes in at least one relative was revealed in 39.3% of the patients.<BR>5. 53.7% of the men and 10.4% of the women smoked, and 36.4% of the men and 3.4% of the women drank.<BR>6.34.4% of the patients were treated with a diet only 36.3% were treated with oral hypoglycemic agents, and 16.0% were treated with insulin.<BR>7. 53.0% of the patients examined suffered from diabetic neuropathy, 39.4% suffered from retinopathy, and 33.5% suffered from nephropathy (proteinuria). Diagnosis of neuropathy was based on diabetes-caused sensory disturbances and on diminished or absent knee reflexes or Achilles reflexes. Diagnosis of retinopathy was based on the presence of diabetes-caused microaneurysm, bleeding, white flecks, new vessels in the retina, or vitreous bleeding.<BR>8. Abnormal electrocardiogram findings were prevalent in 33.2% of the women and in 23.4 % of the men. Myocardial infarctions were found mainly in the men, and ST-T changes were seen mainly in the women.<BR>9. A cardio-thoracic ratio (CTR) of more than 50% was prevalent in 62.3% of the women and in 33.4% of the men.
