Three cases of omphalith (Cullen's umbilical concretion) were reported in a 70 or more year old Japanese female, and two specimens of omphalith were removed and studied. Three old women noticed brownish black verrucous nodule on the umbilical depression one to several years ago, but they hadn't tried to remove them. Then these nodules gradually became to project from umbilical depression. Removed specimens are brownish black stone-like hard on projecting polypoid lesion, but in umbilical depression (cleft), whitish and not so hard. Histopathological examination of these specimens showed no nuclear components of cells, and partly a small quantity of melanin granules by premelanin reaction. It was suggested that omphalith generally consisted of keratinous materials and sebum, and stone-like hard mass tended to was gradually formed in ones which the umbilical depression was deep and didn't clean on umbilicus. Differential diagnosis was also discussed with granuloma pyogenicum, talc granuloma, patent urachus, persistent vitelline duct and polyp, pilonidal granuloma, endometrioma and primary and metastatic tumors of the umbilicus.
