A new semi-synthetic penicillin, carboxybenzyl penicillin or carbenicillin, was evaluated by applying to urinary tract infections. Minimal inhibitory concentrations of this drug against gram negative bacteria isolated from urinary tract infections were determined.<BR>No strain of E. coli isolated was resistant to carbenicilli n. Among 11 Proteus strains one Proteus mirabilis and one Rettgerella were highly resistant to this antibiotic, these two carry R-factor harboring penicillin resistant marker. E. coli K-12 infected with these R-factors are also highly resistant to carbenicillin and ampicillin. However, some Klebsiella and Pseudomonas strains were found resistant to the antibiotic. Serum level of carbenicillin administered by intramuscular injection of one gram reached maximal level of 25 mcg/ml after 30 minutes. Maximum after intravenous administration of 1 g was 80 mcg /m1 at 15 minutes after injection. Half life of serum level of carbenicillin given intravenously was estimated about 30 minutes about 70% and 80% of total amount given were recovered in urine within 6 hours by intramuscular and intravenous administration respectively.<BR>Thirty-three patients were treated with this antibiotic. All of them had been suffered from stubborn urinary tract infections. In 13 (40%) cases satisfactory results were obtained<BR>In 2 of them satisfactory clinical response such as diminishing high fever was obtained though bacteriological cure was not recognized. Nine failure cases (27%) include 2 Klebsiella, 2 Proteus and 3 Pseudomonas infections, other two were mixed infecions with Pseudomonas and Klebsiella strains.
