抄録

Monoclonal gammopathy of undetermined significance (MGUS) is a pre-malignant plasma cell proliferative disorder, characterized by the presence of a serum M-protein and low number of plasma cells in the bone marrow without any evidence of end-organ damage related to multiple myeloma. However, MGUS undergoes a transition to multiple myeloma, AL amyloidosis or another plasma cell dyscrasia at the rate of about 1% a year during follow-up.<br>We encountered a case of MGUS associated with AL amyloidosis diagnosed by morphological and immunological examination. The patient was an 86-year-old woman who had multiple nodules corresponding to the lower lip, the buccal mucosa, the floor of mouth and the soft palate. Histopathologically, the nodules were diagnosed as AL amyloidosis because of positive reaction against IgG κ type antibodies. Furthermore, quantitative analysis of immunoglobulin and immunoelectrophoresis of the patient's urine revealed increased IgG κ type monoclonal protein. Therefore, we carried out bone marrow aspiration and finally diagnosed that this case was caused by ‘MGUS’.
