抄録

The Medical Collaboration and Welfare Office takes a major role at the Tokushima University Hospital in medical collaboration between the university hospital and regional communities.<BR>The office has held various meetings in order to support home care of patients such as regular liaison meetings with Health Care Centers, review sessions of various diseases, conferences to discuss care systems after discharge. We also have given support to patients by contacting each practice division, institution and facility concerned, taking patients' intention into consideration, so that they can receive suitable care in suitable institutions after discharge.<BR>However, it has been revealed in interviews that they had wanted to get more information for nursing and care that we can offer in the university hospital before discharge. I was necessary to provide detailed information and to establish a network among institutions concerned.<BR>We describe here the in which these problems have been solved. The future plans and our goal are also discussed.
