Ten cases of pancreas divisum were found in 335 successful endoscopic pancreato-graphy ; the incidence was 3.0%. Dorsal pancreatogram was obtained by cannulating accessory papilla using narrowed tip cannula in 9 cases and by surgical pancreatography in 1 case. Both of dorsal and ventral pancreatograms as well as clinical features were evaluated in all of the 10 cases. The results obtained were as follows. 1. Abdominal symptoms resembling acute pancreatitis were found frequently in the case of pancreas divisum. 2. Three cases were diagnosed as chronic pancreatitis by evaluation of pancreato-grams. All of the 3 cases had causative factors of chronic pancreatitis other than pancreas divisum itself such as heavy alcohol intake, episode of intracranial operation and obstruc-tion of the Wirsung's duct due to carcinoma of the papilla Vateri. 3. In 2 cases with chronic pancreatitis coexisting with pancreas divisum, irregular dilatation and stenosis were present not only in the dorsal pancreatic duct but also in the ventral pancreatic duct. In 1 case coexisting with carcinoma of the papilla Vateri, the ventral pancreatic duct showed irregular dilatation and the dorsal pancreatic duct was normal. These results suggest that pancreas divisum itself does not necessary predispose to the development of chronic pancreatitis. Careful interpretation of both dorsal and ventral pancreatogram is recommended to evaluate whether chronic pancreatitis coexists with pancreas divisum.
