Application of Magnetic Resonance Imaging to Diagnosis of Radiolucent Foreign Bodies (Peanuts) in the Lower Airway.

Abstract

Diagnosis of a radiolucent foreign body in the lower airway is not always easy. Although Holzknecht's sign through conventional radiography has been considered most useful, it has some difficulties to obtain reliable information for detecting foreign bodies and determining the site of foreign bodies. In order to diagnose more accurately, several imaging methods have been used recently.<BR>We applied magnetic resonance imaging (T1-weighted imaging) to two infant cases with radiolucent foreign bodies (peanuts) in the lower airway for diagnosis and follow-up study. Because a peanut shows a high signal on T1-weighted imaging due to its lipid content with short T1-value and long T2-value, high-intensity masses within the tracheobronchial tree were detected in both cases. In a case with the atelectasis, high intensity mass existed at the oral end of the lesion. Surrounding fatty tissues showing partial volume effect and pulmonaly vessels showing flow related enhancement must be kept in mind as differential diagnosis.<BR>Magnetic resonance imaging is not necessarily recommended as a routine in the diagnosis of tracheobronchial foreign bodies. In this paper its capability in the more difficult cases is demonstrated.
