We presented a case of choledochocele in a 78-year-old female. The patient had been admitted to the hospital again and again for persistent pancreatitis and cholangitis for these 3 years. Endoscopic retrograde cholangiopancreatography (ERCP) failed to visualize because of the sclerotic and stenotic papilla of Vater. Percutaneous transhepatic cholangiography (PTC) demonstrated a cystic dilatation of the end of common bile duct, and the main pancreatic duct which opened directly to the cyst. A diagnosis of choledochocele was made. Since the common bile duct and main pancreatic duct independently and directly opened to the cyst, the excision of the lesion with reconstruction of biliary and pancreatic duct was performed. The wall of cell was covered with the biliary epithelium pathologically.<br> Fifty-nine cases of choledochocele seen in the literature were analyzed and the morphologic and pathologic variations of the disease were recognized. So more particular studies, such as the motility of papilla of each case would be needed for the apprehension of this disease.
