To elucidate the atherogenecity of postprandial lipemia, 15 angiographically defined normolipidemic patients with coronary artery disease (CAD) and 15 normolipidemic healthy controls were recruited for a fat loading test. After administering cream (40g/m2) and Vitamin A (50, 000IU/m2), we monitered the plasma TG, Vitamin A and remnant-like particles (RLP), detected by anti-apo Al and anti-apo B monoclonal antibodies bound-mixed gel, for up to 12 hours.<br>In the CAD group, the TG increased more slowly than in the controls. There was a striking difference in the RLP-C between the two groups. In a fasting state, the RLP positive rate was 20% in the CAD group and 0% in control group (not significant).<br>In a postprandial state, however, the RLP positive rate was 93% and 20%, respectively (p<0.001). The ratio of the RLP-C/RLP-TG was higher in CAD group, suggesting that RLP contained atherogenic lipoproteins which were rich in cholesterol such as chylomicron remnants and β-VLDL.<br>A multivariate analysis revealed that RLP was the strongest (predictive) factor for predicting the CAD. The data obtained indicated that the subjects with imparied postprandial lipemia might be susceptible to CAD.
