We report on the effects of palatal augmentation prosthesis (PAP) on speech rehabilitation after glossectomy.<br>Subjects included 10 males and 2 femalel between the ages of 43 and 63, who were diagnosed as having squamous cell carcinoma or adenoid cystic carcinoma. Eight cases (included cases which were resected a half mobile area of the tongue) had a partial glossectomy, 2cases had a hemiglossectomy, and 2 cases had a subglossectomy.<br>Consruction of a maxillary prosthesis with an augmented palate was done in several steps, a palatal prosthesis with acrylic resin was made, and modeling compound was added to the palatal prosthesis based upon the results of visual inspection and auditory impression. The final shape of the PAP was determined according to static palatograms and auditory impression.<br>Speech improvement was evaluated by a standardized Japanese speech intelligibility test. Each subject was instructed to pronounce each syllable. The recorded pronunciations were transcribed the sounds as Japanese syllables by 5 listners. In addition, 79 glossal sounds of the 100 test syllables were categorized into 5 groups accorging to their lingogram.<br>All cases achieved highter scores by the PAP. The speech intelligibility score (SIS) improved from 52.2±14.5% without PAP to 64.4±10.4% with PAP. The SIS of glossal sounds improved from 42.3±17.4% without PAP to 57.1±12.7% with PAP. In the speech intelligibility to the manner of articulation and the point of articulation, plosives /p//b//t//d/, affricatives /t∫//dz/, nasals /m/, flaps /s/ and vowel /i/ were significantly improved by the PAP (p<0.05). Comparison of SIS of glossal sounds with and without PAP the sounds producted at the blad and mid portion of the tongue were significantly improved (p<0.05).
