Postoperative complications in chronic hemodialysis patients.

Abstract

In order to determine the indications for and safety of surgical treatment for dialysis patients, we assessed postoperative complications derived from 124 surgical procedures excluding blood access in 120 patients undergoing maintenance dialysis. Among the 76 operations performed under general or epidural anesthesia, postoperative complications were observed in 28 cases (37%). There were no correlations between the occurrence of postoperative complications and the patient's age, sex, dialysis history or timing of the last preoperative dialysis. However, all patients with surgical wound infections had diabetic nephropathy. No serious complications were observed after the 48 operations requiring local or conduction anesthesia, regardless of the cause of renal failure. It appeared that, unless the underlying cause of renal failure was diabetic nephropathy, operations could be performed relatively safely even under general or epidural anesthesia. Special care should be exercised in patients with diabetic nephropathy to minimize postoperative complications.
