IDDM mortality in Japan is extremely high compared with that in Western countries. Despite recent improvement in the prognosis of IDDM in Japan, it is still quite poor compared with that in Western countries. We hypothesized that uneven supply of advanced diabetes therapy might contribute to the high IDDM mortality in Japan.<BR>As the part of DERI mortality study, 1, 428 youth-onset IDDM cases diagnosed between 1965-79 were followed unitil 1-1-1990. Based on this hypothesis, we examined the relationship between mortality rates and the per capita number of medical infrastructures (doctors, nurses, hospitals, emergency hospitals, centers for dialysis, pediatricians, diabetologists and dietitians) by prefecture. There were highly significant ecologic correlations between overall, renal disease and acute complication (DKA, hypoglycemia) mortality rates and the availability of diabetologists (p<0.05). There was also a significant correlation between the renal disease mortality rate and the availability of dietitians (p<0.05).<BR>The results indicate a possible contribution of supervision by specialists to the prognosis of individuals with IDDM in Japan.
