Pamidronate is a second generation bisphsphonate used for treatment of hypercalcemia. We report a case of hypercalcemia due to IgD myeloma with renal failure treated with pamidronate. In March 1996, a 74-year-old male developed lumbago, and his serum creatinine was 6.4mg/dl. After 2 months, he was admitted to our hospital for hemodialysis (HD) therapy. His serum Ca concentration was 13.0mg/dl. We diagnosed IgD type myeloma by myeloma cells by bone marrow aspiration and IgD type M protein by serum immunoelectrophoresis. Myeloma kidney was strongly suggested as the primary disease of end-stage renal disease, based on renal biopsy findings. We performed MP therapy for myeloma and administered pamidronate for hypercalcemia. We successfully controlled serum Ca concentration within 9.0-11.0mg/dl. There are few reports of using pamidronate for hypercalcemia in dialysis patients. We consider pamidronate to be effective and safe for treatment of hypercalcemia in HD patients.
