We investigated the difference in the development of neuropathy between IDDM patients with prepubertal onset and those with postpubertal onset. The mean period of HbAic measured and followed up every month was 7.3±2.8years (mean±SD). Several neurophysiological examinations (nerve conduction velocity, orthostatic bloodpressure change, immediate heart rate response to standin [15: 30 ratio], heart rate variation during deep breathingand vibration perception threshold) were performed in 103 patients with IDDM diagnosed before the age of 30 years. According to the mean level of HbA1c, we divided the subjects into two groups, group A (long-term HbA1c<8.0%) and group B (long-term HbA1c≥8.0%).<BR>In group B subjects with IDDM diagnosed after puberty, neurophysiologicalfindings correlated and worsened with the duration of diabetes. On the other hand, those findings did not correlate with the duration of diabetes in group A subjects diagnosed after puberty, or in group A and B subjects diagnosed before puberty.<BR>In conclusion, we suggest that long-term glycemic control influences the development of diabetic neuropathy in IDDM and the influence differs between subjects with prepubertal onset and those with postpubertal onset.
