- A Case of Crow-Fukase Syndrome Diagnosed as Diabetic Polyneuropathy.

A 63-year-old woman was admitted to our hospital because of a progressive disturbance of gait. She was first noticed to be diabetic at 36 years of age and sulfonylurea therapy was started at 45 years of age. At the age of 61, she began to suffer from numbness of the lower limbs, which was not improved by good glycemic control using intensive insulin therapy. Sensorymotor polyneuropathy was rapidly worsened and involved upper limbs and trunk. After admission, clinical evaluations revealed that she was affected with polyneuropathy, hepatomegaly, endocrinopathy (hypothyroidism and adrenal insufficiency), M-proteinemia (IgG-λ type) and skin changes. Plasmacytoma of the left ninth rib was detected and thus, her illness was diagnosed as Crow-Fukase syndrome (CFS) with non-insulin-dependent diabetes mellitus. Although a patient with CFS may present with glove and stocking type polyneuropathy and impaired glucose tolerance, CFS should be diagnosed from motor-dominant polyneuropathy and other characteristics.
