The clinical course of 38 adenoidcystic carcinomas in head and neck.

Abstract

Thirty-eight cases of adenoidcystic cancer of the head and neck were treated at Gunma Cancer Center from 1972 to 1991. The clinical courses of those cases were studied in this series. The primary lesions of 15 cases were in the major salivary gland and 11 cases were in the minor salivary gland in the oral cavity. Ten cases were of the maxillary sinus and 1 case each of the nasal cavity and trachea. The characteristic clinical course of adenoidcystic cancer is very long and lung metastasis occurs considerably after the initial treatment. Among 38 cases, 22 cases had lung metastasis which included 2 cases of lung metastasis diagnosed at the time of the initial examination. Six cases of lung metastasis were observed within 1 year after initial treatment. Seven cases of lung metastasis were diagnosed between 1 to 3 years after initial treatment. Six cases of lung metastasis were found between 3 to 5 years and 3 cases were diagnosed more then 5 years after the initial treatment. The duration after lung metastasis until the patient's death was also relatively lung. Four cases died within 1 year after metastasis was diagnosed, but 7 cases were alive more than 3 years after metastasis. Because of the slow growing nature of adenodcystic cancers, patients suffer longer from this cancer than other head and neck squamous cell cancers. It is therefore more dedebilitating and painful than other head and neck cancers. So it is necessary to be particularly careful in briefing the patient's family when an adenoidcystic cancer is diagnosed.
