We assessed the reliability of Semmes-Weinstein monofilaments for evaluating diabetic neuropathy in 150 patients with type 2 diabetes and 52 healthy subjects. The touch perception threshold (TPT) was measured at the plantar surface of thegreat toe using 3 sizes of monofilaments-4.17, 5.07, and 6.10 bending with 1, 10 and 100g of force.<BR>The TPT did not exceed 4.17 (1g) in 98%(51) of healthy subjects.<BR>The TPT of 11 of the type 2 diabetics wasgrade 6.10 (100g), of 52grade 5.07 (10g), and of 87grade 4.17 (1g). TPTgrades were strongly associated with other neurological functions, such as Achilles tendon reflex, vibration perception threshold, nerve conduction study, current perception threshold, and postural fall in blood pressure. Atgrade 4.17 (1g), patients showed only slight abnormalities in other neurological tests and none had foot ulceration. Atgrade 5.07 (10g), almost all patients had abnormalities in other neurological tests. At grade 6.10 (100g) all patient had severe neuropathy and 90.9% had a history of foot ulceration.<BR>We thus conclude that TPT measurement by Semmes-Weinstein monofilaments is useful for evaluating the severity of diabetic neuropathy.
