Dolichoectasia often causes neurological symptoms, because it presses on cranial nerves and the brain stem. We report a case of dolichoectasia of the vertebral artery. The patient was a 66-year-old woman with hypertension and diabetes mellitus, who presented with dizziness. MRI and MRA revealed compression of the brain stem by elongation of the right vertebral artery. An otoneurological examination was performed and the results of OKP and ETT suggested dysfunction of the middle cerebellar peduncle in the same position as suggested by MRI and MRA.
