Abstract

頭部外傷を伴う頸椎骨折・胸髄損傷患者の嚥下障害を経験した.嚥下障害の原因は,びまん性脳損傷によるものと考えられた.また,カラー長期装着による弊害や誤嚥の経験という心理面も間接的要因と思われた.一般に,脊髄損傷の急性期治療は,呼吸管理が中心で,嚥下障害は見のがされやすい.しかし,今回のように頭部外傷を伴う場合,嚥下障害を合併する例もある.そこで,脊髄損傷患者も嚥下障害に着目する必要があると思われた.

This paper presents a case of dysphagia in a patient with cervical vertebrae fracture, thoracic spinal cord injury, and diffuse brain injury. The causes of the swallowing disorder are mostly attributed to the brain injury, partially to the limited movement of the larynx due to an eight-month immobilization of the cervical spine, and to a psychological factor (the fear of swallowing due to a history of respiratory arrest following aspiration). While respiratory management should be given priority during the acute phase of high cervical cord injuries, this case suggests that swallowing disorders should be taken into account in cases accompanied by brain trauma.
