Abstract

本論文では,重篤な摂食・嚥下障害をもつ患者に対する援助について,実践例を提示し詳細に述べた.今回の症例では重度な障害にもかかわらず,一定の訓練効果を得ることができた.その要因として,我々医療スタッフの諦めない医療実践があったことと,患者本人自身の食べる営みによって,生きる意欲をひき出したことがあったと思われた.このように,ヒトが人として生きていくために「口から食べること」がどれだけ重要な意味をもつかということを再考しながら,STとしての役割と摂食・嚥下障害患者への援助の具体的方法等について私見を述べた.また,摂食・嚥下障害のリハビリテーションにおける「チームアプローチ」のあり方とSTの「プロフェッショナルとしての倫理」に関する2つの提言を行った.

This paper reports in detail on practical training for a patient with severe dysphagia. Though the patient was seriously handicapped, I found that our training was able to bring about continued improvement. Among the factors responsible for this, I discuss how the medical staff patiently continued the medical treatment and training and, by enabling the patient to eat through her own efforts, were able to draw out her desire to live. While reconsidering the importance of “eating” for people to live humanely, I state my personal opinion about the role of the speech therapist and concrete methods to help a patient with dysphagia. I also presented two suggestions concerning the “team approach” for rehabilitating a patient with dysphagia and the “professional ethics” of the speech therapist.
