Abstract

<i>Mycobacterium-avium-intracellulare</i> complex (MAC) 症患者の末梢血単球 tumor necrosis factor alpha (TNFα) と interleukin-6 (IL-6) 産生能とをMAC由来の puried protein derivative-B (PPD-B) 抗原刺激, 非刺激下で測定し, 臨床像との関連を検討した. 非刺激下ではTNFα産生能は活動例, 非活動例, 健常者の順に高く, IL-6産生能は活動例, 健常者, 非活動例の順に高かった. 抗原刺激時では, TNFα産生能は活動例に亢進し, 特に排菌持続期間長期の症例ほど亢進し, 栄養障害の指標である血漿アミノ酸バランスの指標である Fischer 比 (BCAA/AAA) と有意な逆相関が認められた. IL-6産生能は活動例, 非活動例とも健常者に比べ有意な産生低下が認められた. 単球TNFα, IL-6産生能はMAC感染時の臨床病態と密接に関連していると考えられた.

The production of tumor necrosis factor alpha (TNF—) and of interleukin-6 (IL-6) by peripheral blood monocytes (PBMs) from patients infected with <i>Mycobacerium avium intracellular</i> complex (MAC) were assessed.<br>Spontaneous release of both TNF— and IL-6 were greater during the active stage than during the inactive stage and in healthy controls. When the cells were stimulated with MAC-derived purified protein derivative B (PPD-B), TNF— production by PBMs in the active stage increased and IL-6 production by cells in both the active and inactive stages decreased.<br>Moreover, the <i>in vitro</i> increase in TNF— production after stimulation in the active stage seemed to be related to the persistent MAC infection, which resulted in an exhaustion of nutrition.<br>These results suggest that the ability of PBMs to produce TNF— and IL-6 <i>in vitro</i> is closely related to the clinical stage of MAC infection.
