- 一過性血流障害を呈し,bezafibrateにより血清ALP値の著明な改善をみた原発性硬化性胆管炎の1例

Abstract
症例は64歳,男性.硬化性胆管炎としてウルソデオキシコール酸(UDCA)内服にて経過観察されていたが,胆道系酵素の上昇をきたし,精査のため入院.腹部超音波検査(US)にて高エコー,造影CTにて動脈-門脈シャントを内包する低吸収域として描出される区域性の血流障害を呈した.MRCP・ERCPにて肝内・肝外胆管は数珠状変化・帯状狭窄を呈し原発性硬化性胆管炎(PSC)と診断した,また肝生検にて,血流障害区域に周辺とくらべ強い線維化を確認した.3カ月後のCTでは血流異常像は消失傾向を示した.UDCA600mg/day経口投与に加え,bezafibrate400mg/day経口投与追加したところ,ALP値は速やかに低下傾向を示した.
