Abstract

消化器癌術前貯血例において,保存自己血中のエンドトキシン (ET) と炎症性サイトカインとの関連性,および閉塞性黄疸とこれらの値との関係を検討した.対象は閉塞性黄疸合併担癌29例,非合併担癌39例の計68例で,貯血された自己血中ETおよびtumor necrosis factor α (TNF-α), interleukin-6 (IL-6) を測定し,相互間の関連や自己血保存期間との関係を検討した.黄疸群の自己血中ETは,非黄疸群に比し有意に高値を示し,保存期間の延長とともに上昇する傾向を示した.さらに,自己血中 TNF-α はETの上昇とともに高値を示し,両者の間には0.87と高い正の相関を認めた.閉塞性黄疸合併担癌患者では,末梢血あるいは自己血中ETが高値を示す例があり,その結果として炎症性サイトカインが上昇する可能性があると思われた.今後さらに両者の関連性に関し検討が必要と思われた.

In this study, values of endotoxin (ET), tumor necrosis factor α (TNF-α) and interleukin 6 (IL-6) in strage autologous blood were measured in a total of 68 patients with gastrointestinal cancer including those with obstructive jaundice. And the correlations between ET, and TNF-α and IL-6 in autologous blood, and the correlation of them with strage period were evaluated. The values in ET of peripheral blood and autologous blood in obstructive jaundice group were significant higher than those in non-obstructive jaundice group. And the values of ET were correlated with duration of strage period in obstructive jaundice group. The values of ET and IL-6 were positively correlated and the coefficient was 0.69 in all cases, and between ET and TNF-α in obstructive jaundice group was 0.87. It is suggested that there is a possibility that high level of ET in autologous blood induces the production of IL-6 and TNF-α in cancer bearing patients with obstructive jaundice. Farther studies on the correlation between them are needed.
