We experienced two patients with hepatic metastasis developed about 8 years after resection of leiomyosarcoma of the duodenum or small intestine, in whom hepatectomy was successfully performed. In this study, factors that disturbed the growth of metastatic foci in them were investigated. And we reviewed the literature in terms of the correlations among tumor diameter of the primary lesion which is considered useful for the malignancy assessment, mitotic index (MI), and time from the resection of the primary foci to hepatic metastasis. The metastatic hepatic tumors had two characteristic features, namely formation of fibrous capsule and infiltration of cytotoxic T lymphocytes (CD8+T cells). Fibrous capsule might disturb blood circulation to the tumors and induce necrosis of the tumor cells. And cytotoxic T lymphocytes might lead to damage of the tumor cells. Accordingly, the two features might contribute to inhibition of the growth of metastatic hepatic tumors and link with favorable prognosis in the present cases. There was no significant correlation between MI and the time to the hepatic metastasis.
