It is well-known that thymoma is often associated with myasthenia gravis. We have recently experienced a patient with myasthenia gravis with thymoma who was found also having multiple gastric cancers at preoperative work-up and underwent operations on two-step approach.<br> A 74-year-old man on medication for myasthenia gravis was found to have a thymoma on a chest CT scan. In addition, upper gastrointestinal series demonstrated multiple gastric cancers. The patient underwent resection of the thymus with thymoma, followed by a total gastrectomy on two-step approach. Clinical course after the first operation was uneventful and the patient recovered so well that no medication was needed for myasthenia gravis. Six weeks after the second operation, a common cold was followed by a myasthenic crisis. He gradually recovered by artficial ventilation along with steroid pulse therapy.<br> There are reports describing that patients with thymoma may be associated with some malignant tumor other than thymoma in a high frequency. A thorough preoperative examination as well as periodical postoperative check, seems to be recommended.
