Abstract

This paper describes an adult case of Bochdalek hernia causing rupture of the colon and gastric perforation which progressed to its termination. A 61-year-old woman, who had been pointed out having esophageal hiatal hernia several years before but had been asymptomatic, was admitted to the hospital because of epigastralgia, nausea and anorexia. A chest rentogenogram on admission showed a large abnormal gas with Niveau in the left pleural cavity. Magnetic resonance imaging (MRI), coronal and sagital T1-weighted images clearly demonstrated the discontinuity of the posterolateral aspect of the left diaphragm adjacent to herniation of the stomach into the left pleural cavity. Since left atelectasis progressed and rapid exacebation of inflammatory findings occurred, an emergency operation was performed on the 9th hospital day. At operation, the gastric body and an absolutely ruptured colon about 30cm in length protruded into the left pleural cavity. Preoperative MRI findings were well confirmed intraoperatively. Resection of the ruptured colon, direct suture closure of the hernia opening and stomach perforation, and transeverso-descending colonostomy were performed. However, the patient died of septic shock 40 days postoperatively.
