抄録

Primary abdominal hernias, especially spigelian hernia, are rare. We present a case of spigelian hernia associated with lateral inguinal hernia.<br> A 67-year-old woman noticed a left lower abdominal tumor which was felt in the upright position when abdominal pressure was added. Abdominal ultrasound revealed a 3.0×1.5cm hernia sac projecting from left lateral straight muscle of abdomen through the abdominal cavity to subcutaneous space of the abdominal wall, and around 1.5×0.6cm hernia opening. Spigelian hernia was suspected and an operation was performed on March 13, 2000. During the operation, no prominent hernia sac was confirmed, but defects on fascia were present in the musculus obliqquus internus abdominis and musculus trnsversus abdominis, from where the herniation might occur. Since association of left lateral inguinal hernia was found, reinforcement using large size of prolene mesh sheet was made through the same surgical wound. Postoperative course was uneventful. No signs of recurrence of both hernias have been demonstrated.
