Intracranial subdural hematoma following spinal anesthesia is a very rare, but life-threatening complication. We experienced a case of this complication following spinal anesthesia. A 40-year-old man underwent a hemorrhoidectomy under spinal anesthesia and complained of intense headache, light nuchal rigidity and vomiting four days after the operation. CT scan of the brain revealed an acute intracranial subdural hematoma. Without any surgical treatment, the hematoma was gradually absorbed and a complete resolution of the hematoma was demonstrated on the CT scan 50 days after the initial onset of symptoms. No sequela was noted.<br> Headache after spinal anesthesia is a common complication, but when the headache persists, is severe, and is associated with other neurologically symptoms, we should treat the patient by entertaining a possible association of the disease.
