Abstract

下大静脈原発平滑筋肉腫は,比較的発育速度が遅く,悪性度も低いとされているが,外科的切除を行える症例は少ない.今回われわれは,骨転移を伴った下大静脈原発平滑筋肉腫の1例を経験したので報告する.<br> 症例は, 73歳女性.主訴は背部痛.胸部レントゲン写真にて右第6肋骨に腫瘤陰影および骨融解像を認め, CT, MRI,血管造影検査で,下大静脈に接する約3cmの腫瘤を認めた.手術は,腫瘍を含む下大静脈を切除し,径20mmのリング付きゴアテックスで静脈再建を行い,右第6肋骨切除を行った.<br> 病理組織学的所見は,下大静脈原発の平滑筋肉腫と肋骨転移であった.術後7カ月現在,再発の徴候はない.

It is believed that leiomyosarcomas of the inferior vena cava have a relatively low growth rate and a low malignant potential, but few cases of the disease are able to be resected surgically. We present a case of leiomyosarcoma of the inferior vena cava with bone metastasis which was successfully resected.<br> A 73-year-old woman was admitted to the hospital because of back pain. Chest X-ray examination revealed a tumor shadow and osteolysis at the right 6th rib. CT, magnetic resonance imaging (MRI), and angiography revealed a tumor about 3 cm in size in the vicinity of the inferior vena cava. Operative procedures included resection of the inferior vena cava including the tumor, reconstruction of the vein using Goatex graft with a ring 20 mm in diameter, and then resection of the right 6th rib.<br> Histopathologically, leiomyosarcoma of the inferior vena cava and its metastasis to the rib were identified. There have been no signs of recurrence, as of 7 months after the operation.
