Spontaneous bilateral urinary extravasation into the retroperitoneum is relatively rare. A case thereof caused by extensive retroperitoneal involvement of carcinoma of the transverse colon is reported with a review of literature. A 37-year-old male patient who was operated on for mutinous carcinoma of the transverse colon one year previously revisited our clinic with a complaint of dull colicky pain around the right costovertebral angle. A huge urinoma due to the rupture of the renal pelvis was demonstrated. The cause of this condition was considered to be metastatic tumors which obsturcted the lower part of the right ureter. The middle part of the rectum was also affected by the tumors. The metastatic tumors were resected together with a part of the rectum and the right ureter.<BR>One year later, the patient developed a dull pain in the left lumbar region. Left perirenal urinary extravasation which was caused by a metastatic tumor surrounding the left ureter was demonstrated. Left nephrostomy was the only acceptable therapeutic modality for symptomatic relief. Although the incidence of urinary extravasation due to ureteral obstruction is rare, and bilateral urinary extravasation is even rarer, this condition should be kept in mind when treating retroperitoneal metastasis of gastrointestinal carcinoma.
