A 61-year-old man was admitted to the hospital because of severe abdominal pain. On admission the abdomen was board-like and muscle guarding in the entire abdomen was present. Abdominal ultrasonography and CT showed a tumor of the small intestine in the left lower quadrant. The case was diagnosed as diffuse peritonitis with a small intestinal tumor, therefore, emergency laparotomy was performed. The tumor in the small intestine was located at 150cm on the anal side of the Treitz's ligament and was this sounds like the s-bowl was perforated. Partial resection of the small intestine was carried out. The histopathological diagnosis was moderately differentiated adenocarcinoma of the small intestine. The patient was discharged from the hospital on 19th postoperative day.
