The main sites of the teratoma in infancy and childhood are in the sacro-coccygeal, retroperitoneal, mediastinal, ovarial and testicular regions. The stomach is a rare site and gastric teratoma accounts for less than 1% of all childhood teratomas. Gastric teratomas most commomly occur in males under the age of 12 months. This is the 52nd case of gastric teratoma in Japan. A male infant weighing 3.6kg was born by normal delivery on Feb 21, 1994. He was referred to the facility because of a large, firm abdominal mass with respiratory distress. Abdominal sonograms and CT scan revealed both solid and cystic components with coarse, spotted calcifications. Upper gastrointestinal series revealed forward displacement of the gastric lumen due to a tumor. Magnetic resonance imaging was useful in distunguishing the mass from those of retroperitoneal origin. At operation a huge encapsulated tumor attached to the posterior wall of the stomach was removed with a small portion of gastric wall. Surgical specimen measured 14.5×10×6.5cm and weighed 536.5g. The cut section showed solid and cystic areas. Histological examination revealed a mixed germ-cell tumor with mature teratoma and microfoci of immature neuroglial elements. There have been no recurrence, as of 6 years after the operation. The patient is well grown.
