Ten Cases of Residuum of Cancer after Endoscopic Mucosal Resection(EMR) of Early Gastric Cancer.

Abstract

Endoscopic mucosal resection (EMR) has been widely indicated for early gastric cancer recently. However, residuum of cancer after EMR can happen occasionally. Even in such cases, gastrectomy is not the essential requisite. Ten such cases of intramucosal cancer encountred at the hospital from January 1989 to March 1997, were endoscopically followed for 19.28 months in average.<br> Of the ten patients three patients were followed by local injection of destructive drugs, hot biopsy or without any treatment. Cancer was negative 21, 29 and 30 months after EMR, respectively. In other three patients EMR was repeated within 1 week and various local anticancer treatments were added when necessary. Cancer was negative 2, 14 and 34 months afterward. In the remaining four patients, the second EMR was performed over 1 week after EMR and local endoscopic treatments were added on demand. They are free from cancer 12, 38, 42 and 89 months after the first EMR.<br> In the treatment of intramucosal gastric cancers, which are candidates for EMR, even if residuum of cancer is noted after EMR, complete healing can be achieved by repeated local therapies including additional EMR without surgical gastrectomy.
