Transverse Colnic Cancer with Lymph Node Metastasis of Hepatic Hilus Diagnosed by Preoperative Biopsy via an Endoscopic Ultrasound. A Case Report.

Abstract

A rare case of transverse colonic cancer with lymph node metastasis of the hepatic hilus is reported. A 78-year-old woman was admitted for a tumor at the hepatic hilus detected by US examination. Imaging studies suggested a diagnosis, but not definitively, of a tumor derived from lymphatic or mesenchymal tissues of the hepatoduodenal ligament. Therefore, she underwent a preoperative biopsy by endoscopic ultrasound. Although the tumor biopsy specimen consisted of adenocarcinoma, no other obvious findings were observed in the gastrointestinal tract and her ovaries, except a small tumor in the transverse colon. Laparotomy findings disclosed the hard round mass at the hepatic hilus and lymph node metastasis in the mesocolon, and a transverse colectomy with lymph node resection was performed. Histopathologically, the tumor revealed lymph node metastasis of the transverse colonic cancer.<br> Transverse colonic cancer with lymph node metastasis to the hepatic hilus is extremely rare, and preoperative biopsy via endoscopic ultrasound was useful in determining in this case.
