A 59-year-old man was referred to the hospital for evaluation of an abnormal shadow in the right lower lung field on a routine chest roentgenogram. CT scan and MRI of the chest revealed a cystic mass in the posterior mediastinum, although the relationship between the mass and the spinal canal was uncertain. Operation was performed and the mass was found to be cystic and its communication with the spinal canal was suspected. The cystic mass was excised, and diagnosed pathologically as meningocele.<br> Intrathoracic meningocele is a rare entity. There have been only 33 cases including the present case in the Japanese literature. Although intrathoracic meningocele is often accompanied by neurofibromatosis and kyphosis, a few cases, like the present one, without these features have been reported. Therefore it is important to be aware of this entiry when any posterior mediastinal mass is detected.
