This paper describes a case of pulmonary blastoma in a 38-year-old man. He consulted another hospital because of cough and fever. Radiological examination showed abnormal radiopaque shadow which was round in shape (diameter 7.4cm) attached to the upper-anterior chest wall. He had no abnormal shadow at routine chest examination 7 months before. Needle biopsy was performed for pathological diagnosis, by which anterior mediastinal teratoma was confirmed. Thoracotomy was carried out. During the operation, a pulmonary tumor was also found, and then left upper lobectomy and mediastinal-hilar lymph node dissection were performed. However, histopathologically, proliferation of tumor cells was observed, which was composed of ectodermal cells and mesenchymal cells, and resembled the fetal lung. From these findings, we confirmed the tumor as pulmonary blastoma. Two course of chemotherapy with Actinomycin D(AMD) and vincristine (VCR) were added postoperatively.
