A 54-year-old Japanese man was admitted to the hospital with a diagnosis of blunt hepatic injury, which was deep laceration with tissue necrosis, due to a traffic accident. On admission, the patient was hemodynamically stable, without any evidence of peritonitis. He was treated conservatively. Thereafter, his abdomen was distended gradually. On the 20th hospital day, ultrasonograms, computed tomograms and US guided needle aspiration revealed an extrahepatic biloma. A drainage catheter was placed in the biloma percutaneously. However, the biloma extended from the right subphrenic cavity to the floor of pelvis. On the 71st hospital day, the biloma was resected with a T-tube drainage into the common bile duct. The injured site of the bile duct could not be determined at laparotomy. Postoperative cholangiograms performed via the T-tube catheter revealed bile leakage from the right hepatic duct. On the 65th postoperative day, follow-up cholangiograms demonstrated choledocholithiasis and an injured site of the hepatic duct, which was healed conservatively. choledocholithotomy under cholangioscopy through the T-tube fistula was performed for stones in the choledochus. We present a patient with extrahepatic biloma due to a blunt hepatic injury which is very rare, together with a review of the Japanese literature.
